How to get your 329 Card:
Step 1: Schedule an Evaluation
Give us a call, send a text, or drop us an e-mail to set up an appointment with one of our physicians.
Step 2: Meet with the Doctor
The doctor will meet with you to discuss your qualifying conditions and the use of medical cannabis. You will find out at the end of the appointment if you qualify for a 329 card.
Step 3: DOH Application
Hawaii DOH’s Medical Cannabis Registry requires every patient to submit an online application. A current ID is required (does NOT have to be state of Hawaii). The DOH charges a non-refundable application fee of $38.50 that is paid upon submission.
*If you would like help with your application, Lehua Clinic can fill out the application and submit it on your behalf for an additional $25.
Step 4: Receive Card
You will receive and email from the DOH when they process your application. Sign into your Medical Cannabis Registry account to download//print your card.
